About Phipps Neighborhoods
Phipps Neighborhoods, the social service affiliate of Phipps Houses, has been a catalyst for community growth and empowerment for over 50 years. Our mission is focused on creating safe, inclusive spaces where community members can access education, career opportunities, and economic pathways. Serving approximately 14,000 New Yorkers annually, we address barriers to educational achievement, economic security, and career stability through three key program areas:
Education and School-Day Programs: Offering pre-kindergarten services, community schools, after-school programs, and college access counseling.
Sector-Focused Training and Employment Placement Pathways: Providing specialized training, employment placement pathways, Summer Youth Employment, Work Learning Grow initiatives, and aiding residents in accessing new employment opportunities.
Community & Economic Empowerment Services: Enabling community progress through community centers, resident services, food access, financial literacy programs, and support accessing benefits and wellness services.
We collaborate with government, corporate, and philanthropic partners to effect significant and lasting social change, fostering thriving communities.
The Case Manager at the Courtlandt Community site provides case management, outreach and counseling to the residents at this supportive housing facility.
Salary: $48,000 to $52,000 (base salary commensurate with education, training and related work experience)
Duties & Responsibilities:
Conduct a comprehensive intake assessment for all new residents and review the agency’s policies and procedures with them.
Develop treatment plans for all residents.
Maintain concise and accurate resident files.
Monitor residents’ medication
Provide residents with necessary referrals, including emergencies such as hospitalization, detox, etc. Inform residents about on-site, agency and community resources that are available to them.
Participate in case conferences, staff meetings and training sessions and represent the agency at assigned meetings and conferences.
Input data in Salesforce and Maven data bases
Attend professional development trainings, supervision meetings, case conferences, and staff meetings.
Provide monthly statistical reports on time and accurately.
Work cooperatively with staff at all levels to ensure quality service provision and the development and improvement of team performance
Other duties as assigned.
Qualifications:
Bachelor’s Degree preferred in Social Work, Psychology, Sociology, Human Services or related field. Minimum associate’s degree or 60 college credits with two to five years experience.
Qualified candidate must have a strong understanding of the developmental needs of adults, sensitivity to ethnic and cultural diversity, and knowledge of and commitment to working from a strength-based perspective.
Applicant must have excellent written and verbal communication skills, as well as strong computer skills.
Bi-lingual (Spanish) a plus.
